Job Description

We are looking for a dedicated Registered Nurse to join our Intake and Hospital Liaison team at UVM Health Network Home Health & Hospice. In this vital role, you will serve as a critical link between referring providers, program leaders, clinicians, and patients/families, ensuring seamless transitions to our home health and hospice services.

Responsibilities

• Educate patients and referring providers about our services and offerings.
• Coordinate patient discharges and attend rounds as part of the multidisciplinary team.
• Review clinical documentation to assess patient eligibility for services.
• Write orders for care and provide essential information to patients and families.

Requirements

• Associate's degree in nursing and a valid RN license.
• Minimum of two years of nursing experience.
