General Summary

We are seeking a dedicated Case Manager to provide individualized services in vocational, personal, and social skills for our employment participants. The role involves developing service plans, monitoring progress, and coordinating services to ensure the well-being of our clients.

Responsibilities

• Develop and monitor individualized service plans.
• Coordinate services and communicate with team members.
• Provide personal and vocational counseling, crisis intervention, and advocacy.
• Assist with skill development, job modifications, and personal care.

Requirements

• Associate degree in a related field, Bachelor's degree preferred.
• 2 years of experience working with individuals with developmental disabilities.
• Supervisory experience is a plus.
• Valid driver's license, good driving record, and proof of insurance.

• Business acumen and knowledge of developmental disabilities.
• Customer focus and ability to provide positive interactions.
• Drive for results and ability to find solutions proactively.
• Proficient in Microsoft Office and databases.
