Job Description
We are looking for a dedicated Chronic Care Navigator to join our team at Phamily. As a full-time position based in West Columbia SC, you will play a crucial role in supporting our nephrologist in delivering patient-centered care through the Phamily platform. The Chronic Care Navigator will work towards improving patient outcomes by identifying needs, engaging patients, and enhancing communication within the care team.
Key Responsibilities
- Understand nephrology requirements for coordinated population health.
- Collaborate effectively with the care team to establish processes.
- Utilize the Phamily Chronic Care Management platform to support patients with chronic diseases.
- Contribute to quality improvement and care redesign efforts.
- Provide a positive experience for patients through interactions and referrals.
- Report data inconsistencies and support teamwork.
- Ensure compliance with policies and procedures.
This role will involve working closely with various healthcare professionals, attending meetings, and performing tasks to enhance patient care and practice efficiency.
Minimum Qualifications
- At least 1 year of experience in nephrology.
- Proven problem-solving skills and ability to multitask.
- Excellent written and verbal communication skills.
- Certification as a Medical Assistant from a recognized organization.
