INPATIENT CASE MANAGER (RN)

Job Description

We are currently looking for an Inpatient Case Manager (RN) to join our team at LifeBridge Health. This full-time position based in Randallstown, MD, at Northwest Hospital offers a sign-on bonus potential of $15,000. The role involves discharge planning coordination and intervention in collaboration with the clinical team and medical providers to promote patient wellness, improve care outcomes, optimize health services utilization, and minimize denials of payment for patients with complex health needs.

Responsibilities

• Coordinate discharge planning in alignment with patient care goals.
• Collaborate with the clinical team and medical providers to optimize patient care outcomes.
• Promote efficient utilization of health services while ensuring patient advocacy.
• Minimize payment denials through accurate documentation and compliance.

Requirements

• BSN required from CCNE accredited schools.
• Strong professional knowledge with a Bachelor's degree in nursing or related field.
• Prior experience in utilization review, case management, or care coordination preferred.
• Exceptional communication and interpersonal skills.
