Job Description

Join our team at the Agency on Aging of South Central CT as a Care Manager. In this role, you will be responsible for assessing the health, functional, and cognitive status of clients to determine their need for continued home care services. As part of our commitment to community support, you will also educate clients on available resources, assist with applications for services, and evaluate the effectiveness of care plans. Additionally, timely documentation and reporting are essential to ensure clients receive necessary services promptly.

Key Responsibilities

  • Complete annual assessments and paperwork to continue client services.
  • Educate clients on available resources and program services.
  • Assist with applications for supportive services and programs.
  • Monitor and adjust care plans as needed.
  • Ensure timely documentation and reporting.

Requirements

  • Bachelor's degree in health, social work, gerontology, nursing, or related field.
  • Minimum of one year of experience working with older adults or individuals with disabilities.
  • Strong interpersonal and organizational skills.
  • Experience with community resources and state-funded program documentation.
  • Bilingual abilities are a plus.

Compensation

$47,500 yearly salary.
