Job Description

Superior HealthPlan is seeking a Care Manager RN - Foster Care to join our Medical Management/Health Services team. This hybrid position requires someone with pediatric experience and must be licensed and reside in Texas. As a care manager, you will perform duties to assess, plan, and coordinate all aspects of medical and supporting services across the continuum of care for select members to promote quality, cost-effective care. Your responsibilities will include assessing member's health status, resource utilization, developing care plans, coordinating services between healthcare providers, providing patient and provider education, and monitoring referrals to community-based services. You will also participate in integrated team care management rounds, identify risk management quality concerns, enter and maintain assessments, authorizations, and pertinent clinical information into various medical management systems, and more. This role will be the delegated agency's single point of contact to coordinate the overall care management activities provided to the target population, reviewing and approving the initial and ongoing assessment and care plan, coordinating service requests/authorizations, facilitating member linkages to MCO-based services and programs, and more.

Requirements

  • Graduate from an Accredited School of Nursing
  • Bachelor’s degree in Nursing preferred
  • 2+ years of clinical nursing experience in a clinical, acute care, or community setting and 1+ years of case management experience in a managed care setting
  • Current state’s RN license
  • Texas Requirements: 2+ years of clinical nursing or case management experience in a managed care environment, experience working with people with disabilities and vulnerable populations, state-specific requirements may apply
