Job Description
We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. This full-time position at Northwest Hospital in Randallstown, MD, offers a sign-on bonus potential of $15,000. Working 4/10-hour shifts weekly, from 8:30 am to 7:00 pm, the role involves collaborating with the clinical team and medical provider to provide discharge planning coordination and intervention. The Inpatient Case Manager plays a vital role in promoting patient wellness, improving care outcomes, and ensuring efficient health service utilization.
Responsibilities
- Collaborate with clinical team and medical provider for discharge planning.

- Minimize denials of payment among patients with complex health needs.
Requirements
- Bachelor's degree and specialized field knowledge.
- BSN required from CCNE accredited schools.
- Previous experience in a similar role preferred.
