About APM: Asociación Puertorriqueños en Marcha (APM) is a nonprofit organization in Philadelphia, PA, dedicated to providing a wide range of human services to the community since 1970.

Status: Full Time (40 hours per week)

Work Schedule: Mondays through Fridays, 8:30 am to 5 pm

General Summary

The Community Connector will be responsible for conducting community outreach focusing on non-fatal shootings victims and individuals at risk of perpetuating violence in North Philadelphia, working closely with APM's Violence Intervention Therapists.

Responsibilities and Duties

  • Engage with non-fatal shooting victims and at-risk individuals in collaboration with the APM Violence Intervention team
  • Collaborate with key partners to enhance referrals and resources
  • Conduct eligibility assessments and maintain outreach lists
  • Represent APM at various community events and meetings

Requirements

  • BA preferred or equivalent experience with a minimum of three years in direct service work with high-risk young people
  • Experience working with street-involved high-risk youth
  • Valid Driver's License or access to public transportation
