CSC Case Manager

Job Description

PermiaCare is seeking a CSC Case Manager to join our dynamic team. This position involves providing psychoeducation, psychosocial rehabilitation, and case management services to individuals enrolled in the CSC-FEP program. The ideal candidate will work closely with the CSC Program Coordinator, supportive employment specialists, and other team members to ensure individuals receive comprehensive care and support. This role requires a Bachelor's degree in a related field and at least 1 year of experience in social services. The CSC Case Manager will play a vital role in assisting individuals with mental health issues and helping them achieve their recovery goals.

Responsibilities

• Provide direct services to individuals in the CSC-FEP program.
• Work with individuals and families to develop and achieve recovery goals.
• Monitor service delivery to meet monthly requirements.
• Participate in outreach and education efforts about First Episode Psychosis.
• Coordinate services to designated caseload and maintain accurate documentation.

Requirements

• Bachelor's degree in psychology, social work, or related field.
• At least 1 year of experience in social services preferred.
• Valid Texas Driver's license and ability to pass background checks.
• Ability to work independently and collaborate effectively with a multidisciplinary team.
