Job Description
We are looking for a dedicated Home Health Registered Nurse (RN) to join our team at Interim HealthCare of West Palm Beach on a per diem basis. Responsibilities
- Deliver home-based care to patients post-hospitalization or rehab.
- Collaborate with physicians and oversee patient care from LPNs, CNAs, and aides.
- Conduct patient assessments, document progress, and communicate changes to physicians.
- Educate patients and families on care plans, medications, and safety measures.
- Ensure treatment goals are met and coordinate patient discharge.
Requirements
- Minimum 1 year of acute care experience; home healthcare experience preferred.
- Graduate of an accredited nursing program with an active RN license in Florida.
- CPR certification and proficiency in Oasis documentation.
- Strong knowledge of state and federal home health regulations.
- Excellent clinical skills, effective communication, and compassion for patient care.
