The LPN MDS Coordinator plays a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) to ensure optimal patient care and nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Train team members on clinical content and electronic MDS completion.

Qualifications

• Active LPN licensure and good standing.
• Prior experience in healthcare facilities is required.
• Proficiency in MDS and Care Plan completion.
• Knowledge of general nursing practices and regulations.
• Computer skills including Windows applications.
