Job Description

Curana Health, a provider-led primary and post acute organization, is seeking a Nurse Practitioner or Physician Assistant for a Per Diem position in Detroit, MI. The selected candidate will play an integral role in providing compassionate care to Medicare Advantage plan members and their families. This is an opportunity to make a meaningful difference for senior living residents and contribute to the improvement of health and dignity within senior living communities.

Position Responsibilities

  • Performs in-depth health risk assessments on all assigned plan members.
  • Works collaboratively with physicians, Care Coordinators, and other healthcare professionals.
  • Identifies overall quality improvement activities through assessing and planning individualized health care.
  • Reviews and interprets medical, surgical, and social history of members.
  • Performs appropriate screening procedures and interprets diagnostic data.
  • Prescribes or performs various treatment modalities and reconciles medications.
  • Participates in compliance training and seeks guidance for compliance-related concerns.
  • Refers members to PCP, Care Management, and Disease Management programs as necessary.
  • Practices in accordance with applicable laws governing confidentiality and medical information privacy regulations (HIPAA).
  • Provides constructive feedback for operational improvements and maintains a focus on timely customer service.
  • Assists on special projects and participates in client meetings as requested.
  • Communicates and collaborates with facility leadership to achieve member care needs and goals.

Qualifications

  • Valid nurse practitioner or physician assistant license in the state of operations & current board certification in respective field.
  • Masters Degree in Nursing or Physician Assistant Studies.
  • Strong leadership, interpersonal, and problem-solving skills preferred.
  • Excellent written and oral communication skills, punctuality, organizational skills, and ability to function within a team.
