SOCIAL WORKER - CARE MANAGEMENT - CARROLL HOSPITAL

Job Description

We are looking for a dedicated Social Worker to join our Care Management team at Carroll Hospital, part-time with a weekend commitment. The role involves partnering with the healthcare team to ensure efficient inpatient care and coordination with community agencies for various social issues. If you have a Master's degree in social work, 2-4 years of experience, and are licensed or eligible for licensure, we encourage you to apply.

Responsibilities

• Partner with healthcare team members to ensure effective patient care.
• Coordinate with community agencies to address social issues such as abuse, substance abuse, and mental health.
• Link patients to financial resources and support services.
• Possibility of work in the Emergency Department.

Requirements

• Master's degree in social work.
• 2-4 years of social work experience, preferably in a medical setting.
• LCSW-C license preferred, LMSW candidates will be considered.
