Job Description

We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. If you are passionate about providing clinical social work services and psychosocial assessments to patients and their families, this position is for you. As a Social Worker, you will address various issues such as adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Assist patients and families with adjustment to institutional living and financial concerns.
• Offer counseling related to medical issues, aging, discharge planning, and end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
