SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common challenges. This position requires a deep understanding of patient care, financial concerns, family dynamics, and end-of-life issues.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Provide counseling and support for adjustment to institutional living.
• Assist with discharge planning and bereavement support.
• Collaborate with healthcare teams to ensure comprehensive patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of social work experience.
• Licensed Social Worker (LBSW, LMSW, LGPC).
• Strong communication and interpersonal skills.
