Job Description

We are seeking a dedicated Social Worker (PRN) to join our compassionate team at LifeBridge Health in Baltimore, MD. As a social worker, you will provide essential clinical services and psychosocial assessments to patients and their families, addressing a range of common challenges related to institutional living, financial concerns, family dynamics, healthcare understanding, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and families.
• Address issues such as adjustment to institutional living, financial concerns, and family relationships.
• Provide counseling on medical diagnoses, discharge planning, and death and dying.

Requirements

• Bachelor's degree with 1-3 years of social work experience.
• LBSW/LMSW/LGPC certification.
