SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges related to institutional living, financial concerns, family dynamics, medical procedures, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments.
• Provide counseling and support to patients and families.
• Assist in discharge planning.
• Address issues related to aging and end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in social work.
• Licensed as LBSW, LMSW, or LGPC.
• Strong interpersonal and communication skills.
