Job Description

We are looking for a compassionate PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide vital clinical social work services and psychosocial assessments to patients and their families, addressing a range of common challenges such as institutional living adjustments, financial concerns, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and transition to post-healthcare settings.
• Work collaboratively with healthcare professionals to address patients' emotional and social needs.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
