SOCIAL WORKER PRN

Job Description

We are looking for a skilled Social Worker PRN to join LifeBridge Health in Baltimore, MD at Levindale Hospital. In this role, you will provide clinical social work services and psychosocial assessments to patients, focusing on various aspects such as adjustment to institutional living, financial concerns, family relationships, and discharge planning. This position requires someone with 1-3 years of experience and professional knowledge in social work.

Responsibilities

• Conduct psychosocial assessments for patients/residents and their families.
• Offer counseling and support for individuals facing challenges related to healthcare, aging, and end-of-life care.
• Collaborate with healthcare teams to develop effective care plans and discharge strategies.

Requirements

• Bachelor's degree in social work; LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience in clinical social work.
• Strong communication, empathy, and problem-solving skills.
