SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health. As a Social Worker PRN, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life issues.

Responsibilities

• Offer psychosocial assessments to patients and families.
• Provide counseling on a range of topics including discharge planning and bereavement.
• Assist patients in coping with medical conditions and treatment plans.
• Collaborate with healthcare professionals to ensure holistic patient care.

Requirements

• Bachelor's degree and state licensure (LBSW/LMSW/LGPC).
• 1-3 years of relevant experience in a healthcare setting.
• Strong understanding of social work theory and practice.
• Excellent communication and interpersonal skills.
