Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. This position, based in Baltimore, MD at Levindale Hospital, involves providing clinical social work services and psychosocial assessments to patients and families facing various challenges. The role includes addressing issues such as adjustment to institutional living, financial concerns, family relationships, aging-related changes, discharge planning, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide clinical social work services to patients and families.
- Address a wide range of social, emotional, and practical difficulties encountered by patients.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Assist in developing and implementing discharge plans.
Requirements
- Bachelor's degree or equivalent in social work or related field.
- 1-3 years of relevant experience.
- LBSW, LMSW, or LGPC certification.
- Strong knowledge of social work theory and practice.
