Job Description
We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health. As a valuable member of our Allied Health department at Levindale Hospital in Baltimore, MD, you will provide essential clinical social work services and psychosocial assessments to patients, families, and residents.
Responsibilities
- Offer support and counseling regarding various challenges such as adjustment to institutional living, financial concerns, family relationships, aging, and end-of-life issues.
- Assist individuals in comprehending and accepting medical diagnoses, procedures, and treatments.
- Conduct discharge planning and provide guidance on palliative care.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- LBSW, LMSW, or LGPC certification.
- Strong professional knowledge and practical skills in the field.
