SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment issues, financial concerns, family dynamics, and end-of-life counseling.

Responsibilities

• Deliver clinical social work services and psychosocial assessments.
• Address common patient and family challenges related to institutional living, financial matters, family dynamics, and end-of-life issues.
• Participate in discharge planning and provide counseling on death and dying.

Requirements

• Bachelor's degree and 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong knowledge in theory and practice within the social work field.
