SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our dynamic team at LifeBridge Health. In this role, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing a range of common challenges related to institutional living, financial concerns, family relationships, medical procedures, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide appropriate interventions.
• Assist patients and families in coping with emotional and practical aspects of illness.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Coordinate discharge planning and community resource referrals.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in social work.
• LBSW, LMSW, or LGPC certification.
• Strong communication and interpersonal skills.
