SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and skilled Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common issues such as adjustment to institutional living, financial concerns, family relationships, and more. Your expertise will be crucial in assisting individuals through challenging situations and providing counseling and support.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services.
• Address a wide range of patient and family issues including adjustment to care, financial concerns, and end-of-life planning.
• Collaborate with healthcare teams to ensure comprehensive care for patients.
• Participate in discharge planning and providing counseling related to death and dying.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of professional experience in a healthcare setting.
• Licensed as LBSW, LMSW, or LGPC.
