Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, helping them navigate various challenges and transitions. Responsibilities

• Provide clinical social work services to patients and their families.
• Assess common psychosocial issues including adjustment to institutional living, financial concerns, and family relationships.
• Offer counseling and support related to medical diagnoses, aging, discharge planning, and end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as an LBSW, LMSW, or LGPC.
• Strong knowledge of social work theory and practice.
