SOCIAL WORKER PRN

Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common issues. This role involves supporting individuals with adjustment, financial concerns, family relationships, and counseling on medical procedures and treatments.

Responsibilities

• Offer clinical social work services to patients/residents and families.
• Assess and address common psychosocial issues related to institutional living, aging, and discharge planning.
• Provide counseling on a range of topics including family relationships and end-of-life support.

Requirements

• Bachelor's degree and 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong understanding of social work theory and practice.
