Job Description

We are looking for a dedicated Social Worker - PRN to join our compassionate team at LifeBridge Health. In this position, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various challenges they may face.

Responsibilities

• Offer support and counseling to individuals experiencing adjustment issues.
• Assess and address financial concerns affecting patients and their families.
• Provide guidance on family relationships and coping with medical conditions.
• Assist in discharge planning and end-of-life counseling.

Requirements

• Bachelor's degree and relevant professional knowledge.
• 1-3 years of social work experience.
• LBSW/LMSW/LGPC certification.
