Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing various challenges and concerns they may face during their healthcare journey.
Responsibilities
- Conduct psychosocial assessments and offer counseling on a range of issues such as adjustment to institutional living, financial matters, family dynamics, and end-of-life decisions.
- Assist patients in understanding and coping with medical diagnoses, treatment options, and discharge planning.
- Collaborate with healthcare teams to ensure holistic patient care and support.
Requirements
- Bachelor's degree and professional certification as LBSW, LMSW, or LGPC.
- 1-3 years of relevant experience in a healthcare setting.
- Demonstrated knowledge of social work theory and practice.
- Compassionate and empathetic approach to patient care.
