SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various challenges they may face.

Responsibilities

• Offer clinical social work services to address common problems such as adjustment difficulties, financial concerns, family relationships, and end-of-life counseling.
• Conduct psychosocial assessments to understand and support patients/residents and their families.
• Assist with discharge planning and provide counseling related to death and dying.
• Collaborate with healthcare teams to ensure comprehensive care for individuals.

Requirements

• Bachelor's degree and LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience in social work.
• Strong knowledge of social work theory and practice.
• Ability to work with individuals facing various challenges with empathy and professionalism.
