Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common problems such as adjustment to institutional living, financial concerns, and counseling related to death and dying.
Responsibilities
- Conduct psychosocial assessments and provide clinical social work services to patients/residents and families.
- Address adjustment issues, financial concerns, family relationships, and discharge planning.
- Offer counseling related to acceptance of medical diagnoses, aging, powerlessness, and end-of-life care.
Requirements
- Bachelor's degree and 1-3 years of professional experience.
- Licensed as LBSW/LMSW/LGPC.
- Familiarity with social work theory and practice.
