Job Description
We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing a variety of common challenges related to institutional living, financial concerns, family relationships, medical diagnoses, aging, and end-of-life issues.
Responsibilities
- Conduct psychosocial assessments.
- Provide counseling and support related to various social and emotional concerns.
- Assist with discharge planning.
- Collaborate with healthcare professionals to enhance patient well-being.
Requirements
- Bachelor's degree in Social Work or related field.
- LBSW/LMSW/LGPC certification.
- 1-3 years of relevant experience.
- Strong knowledge of social work theory and practice.
