SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker PRN to join our healthcare team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker, you will provide vital clinical social work services and psychosocial assessments to patients and their families, addressing a range of common issues such as adjustment to institutional living, financial concerns, family dynamics, and end-of-life counseling.

Responsibilities

• Offer psychosocial assessments and clinical social work services to patients and families.
• Address various challenges including adjustment to institutional living and end-of-life counseling.
• Collaborate with healthcare teams to provide optimal patient care.
• Assist in discharge planning and help patients cope with medical diagnoses and treatments.

Requirements

• Minimum Bachelor's degree and 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong knowledge and practice within the social work field.
