SOCIAL WORKER PRN

Job Description

We are currently looking for a Social Worker (PRN) to join our team at LifeBridge Health. This position is based in Baltimore, MD at Levindale Hospital, offering a flexible work schedule.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients, residents, and families.
• Address common challenges such as adjustment to institutional living, financial concerns, family dynamics, and end-of-life counseling.
• Support individuals in understanding medical diagnoses, discharge planning, and coping with aging-related changes.

Requirements

• Bachelor's degree or equivalent in social work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
