Job Description

We are looking for a PRN Social Worker to join us at LifeBridge Health in Baltimore, MD. As part of the SOCIAL WORK-HOSPITAL team at Levindale Hospital, you will provide clinical social work services to patients and their families, addressing various challenges they may face during their stay.

Responsibilities

• Conduct psychosocial assessments and provide counseling to individuals and families.
• Address issues such as adjustment difficulties, financial concerns, and end-of-life planning.
• Collaborate with healthcare teams to develop discharge plans and ensure holistic patient care.

Requirements

• Bachelor's degree and relevant professional certification (LBSW/LMSW/LGPC).
• 1-3 years of social work experience.
• Strong understanding of social work theory and practice.
