Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing a range of common challenges such as adjustment to institutional living, financial concerns, family relationships, and more.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and end-of-life counseling.
- Collaborate with healthcare teams to address patient needs effectively.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- LBSW, LMSW, or LGPC certification.
