SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients, residents, and families, addressing various common concerns and challenges they might face. This role involves assisting individuals with adjustment to institutional living, financial issues, family dynamics, medical understanding, aging-related changes, discharge planning, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services.
• Assist patients and families with various challenges and concerns.
• Collaborate with healthcare professionals to ensure comprehensive care.
• Offer counseling and support related to end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed Social Worker (LBSW, LMSW, LGPC).
• Strong understanding of clinical social work theory and practice.
