Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide essential clinical social work services and psychosocial assessments to patients, residents, and families, assisting with various challenges including adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments of patients and their families.
• Provide counseling and support related to medical procedures and diagnoses.
• Assist with discharge planning and bereavement counseling.
• Collaborate with healthcare teams to enhance patient well-being.

Requirements

• Bachelor's degree or equivalent.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Strong interpersonal and communication skills.
