Responsibilities

• Provide psychosocial assessments to patients and families.
• Address common issues related to institutional living, financial worries, family dynamics, and end-of-life care.
• Offer counseling and support for individuals facing medical challenges and life transitions.
• Contribute to discharge planning and assist with end-of-life counseling.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in a healthcare setting.
• LBSW, LMSW, or LGPC certification.
• Strong communication and interpersonal skills.
