SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a social worker, you will provide crucial clinical social work services and psychosocial assessments to patients, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and families.
• Address common problems related to adjustment, financial issues, family dynamics, and end-of-life counseling.
• Participate in discharge planning and assist patients in navigating medical procedures and treatments.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
