SOCIAL WORKER PRN

Job Description

We are currently seeking a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will be responsible for providing clinical social work services and psychosocial assessments to patients and families, addressing a range of common issues related to institutional living, financial concerns, family relationships, medical procedures, aging, discharge planning, and counseling on end-of-life matters.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services.
• Assist patients and families in coping with various challenges and transitions.
• Collaborate with healthcare teams to develop and implement care plans.
• Provide counseling and support related to emotional and psychological needs.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Knowledge of social work theory and practice.
