Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families. Your role will involve addressing a range of issues including adjustment to institutional living, financial concerns, family relationships, aging-related changes, and counseling on discharge planning and end-of-life matters.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common issues such as adjustment to institutional living, financial concerns, and family relationships.
• Offer counseling on medical diagnoses, aging-related changes, and end-of-life matters.
• Participate in discharge planning processes.

Requirements

• Bachelor's degree or equivalent professional knowledge.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
