SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker (PRN) to join our team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common problems such as adjustment difficulties, financial concerns, family dynamics, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and support patients in adjusting to medical conditions and procedures.
• Offer guidance on financial concerns and other challenging issues related to institutional living.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed in Social Work (LBSW, LMSW, LGPC).
