We are looking for a PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common challenges such as adjustment to institutional living, financial concerns, and discharge planning. Responsibilities

• Provide clinical social work services and psychosocial assessments to patients/residents and families.
• Address common problems including adjustment to institutional living, financial concerns, family relationships, and discharge planning.
• Offer counseling related to death and dying.

Requirements

• Basic professional knowledge with a Bachelor's degree.
• 1-3 years of experience in social work.
• LBSW/LMSW/LGPC certification.
