Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, addressing various challenges such as adjustment to institutional living, financial concerns, and family relationships. Your role will involve supporting individuals in understanding medical procedures, discharge planning, and counseling related to end-of-life issues.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Assist individuals in dealing with various challenges such as financial concerns and family relationships.
• Support patients in understanding medical procedures, treatments, and discharge planning.
• Offer counseling and guidance related to end-of-life issues.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
