Job Description

We are looking for a passionate and dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and families, addressing a range of common issues such as adjustment to institutional living, financial concerns, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address common issues including adjustment to institutional living, financial concerns, and counseling related to death and dying.
• Collaborate with healthcare professionals to develop and implement care plans.
• Participate in discharge planning processes.

Requirements

• Bachelor's degree or equivalent in a related field.
• 1-3 years of experience as a Social Worker.
• LBSW/LMSW/LGPC certification.
• Strong communication and interpersonal skills.
