Job Description

We are looking for a dedicated SOCIAL WORKER PRN to join our team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common issues such as adjustment to institutional living, financial concerns, and counseling related to end-of-life care.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Provide counseling and support for individuals facing medical challenges.
• Develop discharge plans and assist with end-of-life care decisions.
• Collaborate with healthcare professionals to ensure comprehensive patient care.

Requirements

• Bachelor's degree and 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong knowledge of social work theory and practice.
• Compassionate and empathetic approach to patient care.
