SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. This position involves providing clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to medical issues and end-of-life care.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Offer counseling and support to patients and families facing medical and end-of-life challenges.
• Participate in discharge planning and coordinate with care teams.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience.
• Licensed as LBSW, LMSW, or LGPC.
