SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our healthcare team at LifeBridge Health. As a Social Worker, you will be responsible for providing clinical social work services and psychosocial assessments to patients/residents and their families. Your role will involve addressing a wide range of common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Assist patients/residents and families in dealing with various emotional and practical challenges.
• Collaborate with healthcare professionals to ensure comprehensive care for patients.
• Conduct discharge planning and offer counseling as needed.

Requirements

• Bachelor's degree and relevant professional knowledge.
• 1-3 years of social work experience.
• LBSW/LMSW/LGPC certification.
