SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, and counseling related to end-of-life issues.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients/residents and families.
• Assist individuals in coping with challenges related to medical diagnoses, aging, and discharge planning.
• Collaborate with healthcare teams to ensure comprehensive care for patients.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC licensure.
• Strong interpersonal and communication skills.
